Introduction

According to the Centers for Disease Control and Prevention, community health assessments pinpoints specific health needs and issues throughout targeted locations (CDC, 2019). By assessing and planning accordingly, there is an allowance to create proactive plans that can improve the health and well-being of communities through community-specific healthcare. When an improvement plan is developed, it entails a systematic process that can promote health and spread awareness regarding public health (CDC, 2019). The purpose of this presentation is to conduct a community health assessment for Dade County Florida and suggest an improvement plan.

Description of Miami-Dade County

The Miami-Dade County is considered the largest metropolitan area within the entire state of Florida (University of Miami Hospitals and Clinics, 2018). As of 2018, there are more than 2.7 million residents with over 60% of the demographic being Hispanic (University of Miami Hospitals and Clinics, 2018). Moreover, 16% of the population is African American, 13% of the population is White and 2% of the of the population is Asian. There are also individuals who are American Indian and mixed race (University of Miami Hospitals and Clinics, 2018).

Currently, there are many health disparities that must be addressed and many of those factors are correlated with the socioeconomic implications as well (University of Miami Hospitals and Clinics, 2018). It has been found that more than 20% of the population near or below the poverty line (University of Miami Hospitals and Clinics, 2018).
